Element Dietetics - Terms and Conditions
Appointments and Consultations:
All appointments and consultations are subject to availability.
Clients must provide accurate and complete information during consultations.
Dietary advice and recommendations are based on the information provided by the client and are intended for educational and informational purposes only. They are not a substitute for medical advice.
Results may vary, and Element Dietetics does not guarantee specific outcomes.
Cancellation and Rescheduling Policy:
A minimum of 24 hours’ notice is required for cancellations or rescheduling.
If a cancellation occurs less than 24 hours before the scheduled session, a cancellation fee of 50% of the consultation fee will apply.
Clients who incur the 50% no-show fee may reschedule their appointment within two weeks of the original booking date. The rescheduled appointment will incur only the outstanding fee.
Late arrivals may result in a shortened consultation time or rescheduling at the discretion of the dietitian.
Element Dietetics reserves the right to cancel or reschedule appointments due to unforeseen circumstances. Clients will be notified as soon as possible.
Payment Policy:
Payment is due at the time of service, unless otherwise agreed upon.
Accepted payment methods include bank transfer, credit/debit card (on site only), and online payment platforms.
A non-refundable deposit of $40 is required for all initial online bookings and meal plan bookings. This deposit will be deducted from the total consultation fee.
Medicare payments can only be processed after full payment of the service fees.
Fees are reviewed annually and may be adjusted accordingly. Clients will be notified of any fee changes prior to their next scheduled appointment.
Privacy and Confidentiality:
All client information is handled in accordance with Australian privacy laws, including the Privacy Act 1988 (Cth).
Element Dietetics uses secure, cloud-based practice management software to store and manage client information, ensuring compliance with privacy and data security regulations.
For Telehealth consultations, an AI transcription software may be used for note-taking purposes only. Your dietitian will inform you prior to the consultation if this software is being used and will request your consent.
Client information will only be used for the purpose of providing dietitian services and will not be disclosed to third parties without consent, except where required by law.
By engaging our services, you consent to the collection and use of your personal information as outlined in our Privacy Policy.
Intellectual Property:
All materials provided during consultations, including meal plans, educational resources, and other documents, remain the intellectual property of Element Dietetics and are for personal use only.
Clients must not reproduce, share, or distribute these materials without prior written consent.
Disclaimer:
Element Dietetics provides nutritional advice and guidance but does not diagnose or treat medical conditions.
Clients with existing medical conditions should consult their general practitioner or specialist before making significant dietary changes.
The information and advice provided by Element Dietetics are based on professional knowledge and best practice, but results are not guaranteed.
Limitation of Liability: Element Dietetics is not responsible for any injury, illness, allergic reaction, or adverse health outcome resulting from the implementation of dietary recommendations. Clients assume full responsibility for their choices and agree to seek medical advice if needed.
General Terms:
These Terms and Conditions are governed by the laws of Victoria, Australia.
Element Dietetics reserves the right to amend these terms at any time. The latest version will always be available on our website.
